Registration Requirements for students enrolling into our Onsite or Virtual Middle and High school programs.

  1. Certified copy of Birth Certificate

  2. Student Health Examination (HRS-H Form 3040) or waiver form  (Valid for onsite students)

  3. Florida Certificate of Immunization (HRS Form 680) or waiver form  (Valid for onsite students)
